PowerUP! Business Assistance Providers

Every PowerUP! participant must consult with a "Business Assistance Provider." See the documents below for a list of providers and to download the form verifying you had a consultation

Upload your signed Business Consultation Form as the last page of your plan, in the Appendix. OR you may instead submit the form to the attention of the PowerUP! Competition by fax to 718.222.5679; or in person; or by mail to PowerUP! Competition, Brooklyn Business Library, 280 Cadman Plaza West, Brooklyn, NY 11201 by 6:00 pm September 10, 2012.

You will be responsible for making an appointment with the organization of your choice and having them sign the form.
